Access Plan Costs 

 

This section is meant to be an overview of the plans. Please refer to the specific contract of your plan for details.

 

The annual access fee provides access to the healthcare services provided by Mountain View Family HealthCare, P.C.

The plan agreements are annual and payment is due at the time of enrollment.

 

Enrollment fee: $99 per family

Payment Options:

  1. Paid in full by cash, check, or credit card.
  2. Paid quarterly. Payments are due Jan 5th, April 5th, July 5th and October 5th. There is an additional $10 billing fee for the quarterly payment plan, but if we receive payment on or before the first of the month due, no bill will be mailed to you and the billing fee will not be assessed. We recommend putting Mountain View on a bill payment program available from your bank to avoid billing fees.

Annual Access Plans:

Plan A-For those of any age, but meant for individuals eligible or on Medicare and those with chronic health care issues.

Annual fee $900 per person per year, $225 quarterly.

Includes the ability to schedule an annual wellness visit, which will be to provide you with an individualized plan on how to achieve wellness at your current time and place in life. There is no fee for this particular exam.

Office visits, Phone consultations and Email visits are $20 each. Other procedures are listed on the fee schedule in your agreement.

I accept patients who are on Medicare under private contract terms. Any services provided by Mountain View are not reimbursable by Medicare. Services offered outside my office are covered by Medicare, even if ordered by me, as long as you are not on a Medicare HMO program.

If you choose this plan and are not covered by Medicare, services provided by Mountain View may be reimbursable through your health insurance. These services will be considered “out of network.” Most medical services provided outside of Mountain View (e.g., labs, x-rays and hospital charges) will continue to be covered by your health plan. Documentation will be provided to at the end of office visits if you wish to request reimbursement from your insurance company.

Annual fees are non refundable and not reimbursed by insurance. Talk to your tax advisor regarding applying this fee to your medical expenses tax deductions.

 

Plan B- this plan is currently only being offered to current, established Mountain View patients and their family members ages 64 and younger. Its purpose is to encourage healthier individuals to invest in wellness, thereby hopefully preventing future illness. It also provides access to a physician at costs generally less than other outpatient facilities when illness does strike.

Annual Access Fees per person or family:

  • $75 quarterly, $300/year/person ages 25-64 (Amounts to $25/month/person for each member)
  • $200 annually per person for ages 2-24 years.

Maximum access fee for a family will be $1000 annually.

This includes the ability to schedule an annual wellness visit, at a reduced cost of $50 per enrolled member. This visit will be to provide you or your family member with an individualized plan on how to achieve wellness at your current time and place in life. It also encompasses a well child visit or school physical for members ages 2-24 years.

Office visits will be billed using the traditional fee levels recognized by insurance, so you will be more likely to receive reimbursement from your insurance company. This fee schedule will range from $65 to $150 for an office visit.
