Plan A, "The Low-cost-per-visit Plan," includes $30 office visits, no matter what the complexity or length of visit. E-mail consultations are $20 and scheduled phone consultations are $30. The ability to schedule one annual wellness exam is included for no additional charge. This plan also offers significantly discounted office procedures, such as joint injections to reduce arthritis pain, trigger point injections to reduce muscle pain, and other procedures.  

Plan B, "The Reduced Fee Access Plan," offers a lower access fee and one wellness examination at a reduced fee per year. If additional office visits or procedures due to illness are needed, these are offered at a reduced fee which is due at the time of service. Forms are provided at the end of these visits for patients to submit to their individual insurance plans for possible reimbursement.
