HOW THE PLAN WORKS WITH REGULAR INSURANCE PLANS

Patients with any insurance can enroll in the practice, but your insurance plan is between you and your insurance company. Dr. Lewis does not contract with any insurance plans, and is therefore considered an "out of network provider."

After you pay for services provided Mountain View, we will give you a form that you then mail to your health insurance company. Your insurance company then determines your reimbursement based on the terms of your plan. You will continue to give your insurance information to the lab, specialist office, or hospital for services outside of Mountain View.

Certain HMO’s usually require you to have an “in network provider” for referrals outside of Mountain View to be covered. Be sure and check your plan’s details regarding their practices, as this is your responsibility. The annual access fee itself is not covered by any insurance.
