Who should sign up for Plan A?

This plan is available to those of any age, but is ideally meant for individuals on Medicare, and individuals with chronic health care issues that require frequent visits with the physician.

This plan may also appeal to individuals and families that budget their healthcare cost and prefer predictable costs for outpatient medical care.

Included in Plan A:

The ability to schedule one wellness visit, which will provide an individualized plan on how to achieve wellness at your current time and place in life. No additional payment at he time of the annual wellness exam is required.

All office visits and phone consultations are $30 each. E-mail visits are $20 each. Home visits, offered when medically indicated, are $125/visit. Home visits for a first time patient will be $200 for the first visit.

Other office procedures, such as mole removals, joint injections, laceration repairs, and other procedures are listed on the fee schedule in your agreement.

Cost of Plan A:

ADULTS: Annual fee $960 per person per year, $240 quarterly.

CHILDREN (ages 0-24 years): (With one adult member) $600/year per child.

Plan A Payment Options:

1. Paid in full by cash, check, or credit card.

2. Quarterly Payment Plan: by cash, check, or credit card. Payments are due Jan 5th, April 5th, July 5th and October 5th. There is an additional $10 billing fee for the quarterly payment plan, but if we receive payment on or before the first of the month due, no bill will be mailed to you and the billing fee will not be assessed. We recommend putting Mountain View on a bill payment program available from your bank to avoid billing fees.

Mountain View Family HealthCare accepts patients who are on or eligible for Medicare under private contract terms. Any services provided by Mountain View are not reimbursable by Medicare. Services, blood work, prescriptions and x-rays offered outside Mountain View are covered by Medicare, even if ordered by Dr. Lewis, as long as you are not on a Medicare Advantage (HMO) program.

Annual fees are non refundable and not reimbursed by insurance. Talk to your tax advisor regarding applying this fee to your medical expenses tax deductions.
