Who should sign up for Plan B?

This plan is offered to individuals and families ages 64 and younger. Its purpose is to encourage healthier individuals to invest in wellness, thereby preventing future illness.

It also provides access to a physician at costs generally less than other outpatient facilities when illness occurs.

Included in Plan B:

This plan includes the ability to schedule an annual wellness visit, at a reduced cost of $50 per enrolled member due at the time of the exam. This visit will provide each family member with an individualized plan on how to achieve wellness at your current time and place in life. It also encompasses a well child visit or school physical for members 0-24 years of age.

Individual office visits will be billed using the traditional coding and format recognized by insurance, so you will be more likely to receive reimbursement from your insurance company. This fee schedule will range from $65 to $150 for an office visit, with the majority of visits costing $85 to $110.

Services provided by Mountain View may be reimbursable through your health insurance. These services will be considered &quotout of network."

Most medical services provided outside of Mountain View (e.g., labs, x-rays and hospital charges) will continue to be covered as described by your health plan. Documentation will be provided to at the end of office visits if you wish to request reimbursement from your insurance company.

Our office can provide guidance on how to file a claim with your insurance company.

Cost of Plan B:

• $360/year per person ages 25-64 (Amounts to $30/month/person for each member) Please note: the quarterly payment plan requires a minimum of $360 due at time of enrollment.
• $240 annually per person for ages 0-24 years.

Plan B Payment Options:

1. Paid in full by cash, check, or credit card.

2. Quarterly Payment Plan: A minimum of $360 is due at the time of enrollment. The remaining portion due will be divided into quarterly payments and are due Jan 5th, April 5th, July 5th and October 5th. There is an additional $10 billing fee for the quarterly payment plan, but if we receive payment on or before the first of the month due, no bill will be mailed to you and the billing fee will not be assessed. We recommend putting Mountain View on a bill payment program available from your bank to avoid billing fees.

Annual fees are non refundable and not reimbursed by insurance. Talk to your tax advisor regarding applying this fee to your medical expenses tax deductions.
